Consultant on the development of a model of a patient school (SOP) for centers of continuous care for people with NCDs based on primary health care centers (educational and scientific direction)

Introduction

The Ukrainian-Swiss project “Reducing the Prevalence of Non-Communicable Disease (NCD) Risk Factors in Ukraine” aims to reduce the burden of disease and prevent premature mortality from NCDs. The project is implemented with the support of Switzerland through the Swiss Agency for Development and Cooperation (SDC).

One of the Project’s activities is aimed at strengthening the capacity to implement interventions to reduce NCD risks, change behavior, and improve knowledge and attitudes towards health.

Within the framework of this activity, it is planned to create and pilot advanced centers for continuous care for people with NCDs at primary health care centers, including the organization of schools for patients with NCDs.

The Project activities cover seven pilot regions (Dnipropetrovska, Poltavska, Lvivska, Rivnenska, Cherkaska, Khersonska and Khmelnytska oblasts) where it is planned to pilot schools in 10 advanced centers.

Place of service delivery

The services under this Terms of Reference will be provided in Ukraine, in Kyiv, with the possibility of remote work.

Tasks

The task of the Consultant is to develop a model of a patient school (SOP) for centers of continuous care for people with NCDs based on primary health care centers (educational and scientific direction):

  • Evaluation of the best international practices in the field of NCD patient education (self-monitoring, use of medical devices at home, lifestyle changes, etc;)
  • Preparation of a model of a school for patients (SOP) for centers of continuous care for people with NCDs based on primary health care centers;
  • Reviewing the lists of necessary medical and non-medical equipment and specifications for use in patient schools;
  • Preparation of the standard operating procedure of the patient school (SOP) of the centers for continuous care for people with NCDs at the primary care level;
  • Preparation of the section on training of organizational managers and nurses for implementation in the curriculum for managers and nurses;
  • Providing specialized consultations;
  • Supporting the piloting of the patient school model in selected primary care centers.

Qualifications

The consultant should have the following knowledge and skills:

  • Master’s degree/specialist’s degree in healthcare; a degree in healthcare will be an advantage;
  • In-depth knowledge in the field of health care, including the application of standards of care in the field of NCDs and their prevention, relevant recommendations of international organizations and professional associations;
  • Experience in developing NCD documents, experience in medical education;
  • Fluency in Ukrainian language. Knowledge of English will be an advantage.

Cooperation with the project office and reporting

The consultant on the development of the model of the patient school (SOP) of the centers of continuous care for people with NCDs at the primary health care centers (scientific and educational direction) will provide services in cooperation with the coordinator of this component and other experts (international, national and regional).
